Employees are required to pay either the calculated percentage or 1.5 percent of salary, whichever is higher. Note:

1) The below Online Calculator provides a calculation based on mandated percentages; it does not provide a calculation based on individual 1.5 percent of salary.

2) Keep in mind that you are responsible for contributing the higher of the resulting calculated percentage or 1.5 percent of monthly salary.
